19 STEPS
1 Open the navigation tree for your patient
2 Click Assessment Plans
3 The Assessment Plan must already be created. If you haven't completed this step, please see the tutorial for "Creating an Assessment Plan"
Select the row of the Assessment Plan
4 Click +New
5 Click Score Sheet
6 Give your Assessment Plan Score Sheet a Name. Remember this is what you or your therapists will be scoring.
7 After providing a Description click on Score Sheet Items
8 Click +Add
9 The Assessment Plan will appear in the pop up window. Individually select the items for the score sheet (Recommend keeping score sheets to around 50 items if scoring on Scout).
If planning to score in Post Facto, you can select all. Click Select ALL
10 Make individual selections from this list by clicking the boxes next to the items (or select-all if using Post Facto) then Click Select
11 Click >Save
12 This is the reminder that Scout users should keep their score sheets to 50 items due to performance concerns with score sheets over that size. If using Post Facto for scoring Assessments, score sheets can be larger. Click Yes
13 At this time, the Score Sheet has been created and is saved. If you wish to assign this to a team or a therapist, Click Assign
14 Assign an assessment score sheet by selecting the Assignment type: Team or therapist, Assigning to, then selecting a date that this assignment will be available. Click green highlight to manage the date (Recommended)
15 The Assignment Date will determine how long this assignment remains available for Assignment lists in Scout. When the assignment expires, it becomes unavailable in the Assignment list for Front Line staff. It can be reassigned if needed.
16 Having an assessment open for about 30 days should be enough time to complete the assessment and then it can be removed from the Assignment list. Click 31
17 Click Assign
18 Use the Breadcrumbs to return back to the Assessment Plan list. Click Assessment Plans
19 That's it. You're done.
