Scoring Assessment Score Sheets in the Collect module using the Post Facto options allows users to create and score on larger than usual score sheets. In addition to learning how to score an assessment in Post-Facto this guide is also going to show you that you can work on filling in the assessment scores over multiple sessions by saving your progress and waiting to lock the scores until you are ready to complete the assessment.
1 Here is the Assessment Plans page for an individual patient
2 This patient already has an assessment plan created; it's the item with the blue indicator. There is also an assessment score sheet prepared; it's the item with the green indicator.
3 This patient's assessment is ready for scores to be entered with the assessment and score sheet set up so click on the patient's initials to display the tree of options so you can navigate to the Collect module
4 Click Collect
5 Click
6 Check the box next to the assessment score sheet you need to enter scores for, then Click Collect Post-Facto
7 Click Session Date:
8 Click the Calendar button to choose the date of the assessment
9 Click the drop down arrow for the session time field to scroll to the time needed
10 Click Collect Data
*Note: Remember this date and time. Later you will learn that you can continue a post facto session that has not been locked by re-entering this information
11 Click on the tabs to move between the assessment's Domains
12 Here I'm opening the Academics activity to score the skills that fall within this domain
13 Enter the scores for your assessment using the scoring buttons (these vary in appearance due to the various scoring criteria among assessments)
14 Click the star if you need to mark the item as mastered
15 If you need to save the progress on your assessment and finish scoring your assessment at a later time click Save
16 Clicking Close will close the saved assessment
17 Click Yes to confirm that you need to close the assessment without locking it
18 To re-open your partially completed assessment select the assessment score sheet and click on Collect Post-Facto
19 Click Collect Data after entering the same date and time from when you originally started scoring the assessment
20 I'm navigating back to the Academics domain to show that the scores are saved from my previous session
21 See the previously entered scores?
22 Continue scoring your assessment
23 When you are completely done entering the assessment scores click on Lock Session
24 Click Lock Session
25 You are done scoring your assessment score sheet! After locking you are redirected back to the Collect module score sheet list
