Therapy Note Templates for Scout and Post Facto

Clinical and Suite customers have the ability for session summary notes in Scout and Post Facto to include a note template. The Pathfinder system will provide all Scout or Post Facto users SOAP Notes as a default template.
NOTE: The template does not have to be used. But, LEARN how to customize the template at the company level, below.
This is an enhancement to the Therapy Note feature from PHI i-Spy, available to our Suite customers and limited to use only in the Practice Schedule.
NOTE: Customers who have already created a template to be accessed in the Practice Schedule will automatically have the same template option available in Scout or Post Facto without additional steps.
Considerations for use of Therapy Note Templates:
- Permissions: There are no permissions associated to the ability to use Therapy Note Templates.
- If a user is able to use Scout or Post Facto, the option to use the template will be available.
- Company Setting: Without making any changes to your company setting, the following default text will appear when “Use Therapy Notes Template” is selected in either Scout or Post Facto.
If your organization needs to change the default message, an Admin must do the following:
1. ACCESS Admin> Company> Settings. If your company is using the default text provided
by Pathfinder, the Setting will not appear on this list page by default.
2. CLICK to open the tool for adding a new Customer Setting.
3. ENTER the following information:
- Setting: From the drop down list, CHOOSE the following item: “CUSTSETTING_THERAPYNOTESTEMPLATE”
- Description: This field is view only. It provides information about what the setting does.
- Value: Users can enter the text they would like to appear in the Therapy Note.
- Data Type: Choose “Text” from the the drop down list.
NOTE: If using therapy notes only in Scout or Post Facto, the use HTML code is not necessary when adding the template into the setting. Using the template in the practice schedule does require use of HTML coding.
4. CLICK Add.
Accessing the Therapy Note Template in Scout:
Use of the Therapy Note Template is optional. A user needs to select the button
to see the template. If the button isn’t selected, the template will not appear and Session
Summary Notes can be entered as usual.
1. When locking the session, in the area where the Session Summary Notes are entered,
CLICK the “Use Therapy Notes Template” button in the lower left.
The default SOAP Note template will appear unless an admin has created the custom template through “Company Settings” outlined above.
2. ENTER notes using the template. The cursor can be used to erase areas.
3. CLICK “Continue” to proceed to the signature page.
NOTE: Clicking “Score Sheet” in the upper left will return a user to the Score Sheet and will clear out the template.
Accessing the Therapy Note Template in Post Facto:
- From the “Collect” area of the system, enter data from a Post Facto session and
CLICK - On the “Session Lock” window, CLICK “Use Therapy Notes Template” in the lower left corner.
The default SOAP Note template will appear unless an admin has created the custom template through “Company Settings” outlined above.
2. ENTER notes using the template.
3. CLICK “Lock Session” to complete the locking process.
NOTE: Press “Cancel” to clear the template and return to the Post Facto data collection screen.
Viewing and Reporting on Therapy Notes
The Therapy Note Template entered from Scout or Post Facto will appear in all reports where the session summary notes are visible.
These reports include:
- Goal Plan Summary Report
- Score Sheet Summary Report
- Score Sheet Summary by Session Report
- NEW: Session Summary Export
